So, after months of work, I've got my application together and approved for my EMT-Intermediate course. I've got my vacation approved, the client notified, and my two weeks absence approved for my Firefighter I course. I've got a room reserved in Conway for that time. I've got the family convinced to take a vacation of their own for that time... They are going to Florida. They can't really come to Conway as most nights I will need to study hard, but I feel bad about them having to stay home while I'm elsewhere, pursuing elective interests.

The classroom and lab parts of the course will be from March 7 - 18 in Conway, NH. The school used to be part of SOLO, the school where I've taken all of my significant medical training before now. They spun off eight or so years ago, and now teach only Intermediate and Paramedic courses while SOLO only teaches First responder and EMT-Basic.

I'm starting to feel a little nervous, and not only because of learning IV skills with, on, and from a lab partner.

The road to Vermont certification is long and hard.

First, I have to complete the class.
Next, I have to complete clinicals at the local emergency room. This will be hard and time-consuming in and of itself; the DOT standards follow. (Why, you ask, does the DOT regulate EMTs? Good question.)
After that, I have to take the National Registry test in New Hampshire and pass. This is a written and practical test.
Then, I have to wait to be certified in New Hampshire. New Hampshire is a National Registry state; Vermont is not.
After that, I have to petition Vermont for reciprical consideration. This is just paperwork and will only add a few weeks of gathering signatures and waiting to the process.
After that, I have to take the Vermont written and practical. After passing the National Registry, this should be easyish since Vermont supports a subset of the National Registry skills.

Ayup.

So, next fall, bog willing, I may be an EMT-I.

One interesting thing is that I will be able to perform different skills depending on where in Vermont I am responding. In the next town over, my primary squad, I will be able to perform a certain number of skills. In my own town, my secondary squad, I will be able to perform more skills since the transporting squad that serves my town and with which I am affiliated is from New Hampshire and can operate under the national protocols.

Yes, it's dumb.

Here are the national clinical standards for EMT-I. They look like fun, but will take a lot of time in my local podunk emergency department.

PSYCHOMOTOR SKILLS
The student must demonstrate the ability to safely administer medications.
The student should safely, and while performing all steps of each procedure, properly administer
medications at least 15 times to live patients.
The student must demonstrate the ability to safely perform endotracheal intubation.
The student should safely, and while performing all steps of each procedure, successfully
intubate at least 5 live patients.
The student must demonstrate the ability to safely gain venous access in all age group patients.
The student should safely, and while performing all steps of each procedure, successfully access
the venous circulation at least 25 times on live patients of various age groups.
The student must demonstrate the ability to effectively ventilate unintubated patients of all age groups.
The student should effectively, and while performing all steps of each procedure, ventilate at
least 20 live patients of various age groups.

AGES
The student must demonstrate the ability to perform an advanced assessment on pediatric patients.
The student should perform an advanced patient assessment on at least 15 (including newborns,
infants, toddlers, and school age) pediatric patients.
The student must demonstrate the ability to perform a compressive assessment on adult patients.
The student should perform an advanced patient assessment on at least 25 adult patients.
The student must demonstrate the ability to perform an advanced assessment on geriatric patients.
The student should perform an advanced patient assessment on at least 15 geriatric patients.

PATHOLOGIES
The student must demonstrate the ability to perform an advanced assessment on obstetric patients.
The student should perform an advanced patient assessment on at least 5 obstetric patients.
The student must demonstrate the ability to perform an advanced assessment on trauma patients.
The student should perform an advanced patient assessment on at least 20 trauma patients.
The student must demonstrate the ability to perform an advanced assessment on psychiatric patients.
The student should perform an advanced patient assessment on at least 10 psychiatric patients.

COMPLAINTS
The student must demonstrate the ability to perform an advanced assessment, formulate and implement
a treatment plan for patients with chest pain.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 15 patients with chest pain.
The student must demonstrate the ability to perform an advanced assessment, formulate and implement
a treatment plan for patients with dyspnea/respiratory distress.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 10 adult patients with dyspnea/respiratory distress.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 4 pediatric patients (including infants, toddlers, and school age) with
dyspnea/respiratory distress.
The student must demonstrate the ability to perform an advanced assessment, formulate and implement
a treatment plan for patients with syncope.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 5 patients with syncope.
The student must demonstrate the ability to perform an advanced assessment, formulate and implement
a treatment plan for patients with abdominal complaints.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 10 patients with abdominal complains (for example: abdominal pain,
nausea/vomiting, GI bleeding, gynecological complaint, etc.)
The student must demonstrate the ability to perform an advanced assessment, formulate and implement
a treatment plan for patients with altered mental status.
The student should perform an advanced patient assessment, formulate and implement a
treatment plan on at least 10 patients with altered mental status.

TEAM LEADER SKILLS
The student must demonstrate the ability to serve as a team leader in variety of prehospital emergency
situations.
The student should serve as the team leader for at least 25 prehospital emergency responses.

The good part about being one of the only four EMTs in east, west, and central nowhere is that I've already got that last requirement.
